Access Florida - Food Stamps, Temporary Cash Assistance, and Medicaid

The Food Stamp Program helps people with low income to buy healthy food. The application can be done online. Please see the website for your local service center. The application is also used for Temporary Cash Assistance, Medicaid, and all other Dept of Children and Families services.

You can view the application in English, Haitian-Creole, or Spanish.

Covenant Hospice

Covenant Hospice provides services to those facing end-of-life issues. They have services for family members as well. Help is available 24 hours a day. Please see the website for more info.

The state of Florida offers health insurance for children from birth through age 18, even if one or both parents are working. It includes four different parts.

Low-Income Medical Clinic - Crossroads Medical Clinic

Crossroads delivers quality health services to low-income, uninsured adults.

To be eligible for care, you must be a patient without health insurance. You must be a resident of Okaloosa or Walton County. Your household income cannot exceed 200% of the Federal Poverty Guidelines. See checklist for additional items.

If you qualify, you will be given an appointment. Crossroads is not a walk-in/immediate care clinic. Each patient will be seen by appointment only. Please call for more information.

Lutheran Services Florida - Ryan White II AIDS Program

Lutheran Services Florida provides the Ryan White Title II AIDS Program. This program is for people living with AIDS. The goal is to improve the quality and availability of health care and support services for people living the HIV disease.

The LSF Ryan White Title II Program provided qualified individuals with referrals to healthcare professionals and other services including:

  • Case Management Services
  • Dental Care
  • Emergency Assistance
  • Housing Assistance
  • Medical Care
  • Mental Health Assessment and Treatment
  • Nutrition Counseling
  • Transportation Assistance

Opportunity, Inc. Okaloosa Walton Homeless Continuum of Care - Health Care

The Health Department sponsors Opportunity Health. Opportunity Health is a primary care facility. It is open to the homeless who can give reliable contact information. Opportunity, Inc. also sponsors a flu shot clinic each fall. Homeless and volunteers serving the homeless can get free flu shots. The flu shots are given by a contracted registered nurse. Homeless persons that need prescription medication are served through one-time vouchers. These vouchers are given through the Mental Health Association of Okaloosa and Walton Counties. Opportunity, Inc. Okaloosa Walton Homeless Continuum of Care - Veterans Programs

Opportunity, Inc. may be able to help veterans with a range of programs. They provide employment training and opportunities. They offer these services for female veterans and veterans with families. They do this through the Department of Labor Homeless Veterans Reintegration Program. The agency also works with others to host a Stand Down for homeless veterans.

Vets will receive:

  • A hot meal
  • Backpacks
  • Bikes
  • Blankets
  • Clothing
  • Legal services
  • Linkages to benefits
  • Medical and dental services

PanCare of Florida, Inc.

PanCare of Florida, Inc. is a non-profit organization. They are based in Panama City, Florida. They open Community Health Centers in Bay and Walton counties. They primarily serve people on Medicaid, Medicare, and Medipass. They also accept sliding scale for those who do not have insurance. You must bring proof of household income for sliding scale. Their Community Health Centers provide primary and urgent care. They also offer dental care.
