Access Florida - Food Stamps, Temporary Cash Assistance, and Medicaid

The Food Stamp Program helps people with low income to buy healthy food. The application can be done online. Please see the website for your local service center. The application is also used for Temporary Cash Assistance, Medicaid, and all other Dept of Children and Families services.

You can view the application in English, Haitian-Creole, or Spanish.

Children's Medical Services - Jacksonville Service Area

This State agency provides specialty medical care for children ages 0-18 years. It is for children with chronic medical conditions that may hinder normal growth and development. Appointments are made by phone. Income limits are based on the age of the child. Please call for an income review.

Duval County Health Department - Boulevard Comprehensive Care Center

This health center offers care and testing for HIV/AIDS and sexually transmitted diseases. All services are confidential. Anonymous HIV testing is available. The center also houses the Refugee Health Clinic, Mental Health services and more. Please see the website for more information.

Duval County Health Department - Women's Health Services

Women should receive annual examinations for early detection. The health department offers regular tests such as mammograms and Pap tests. Women can also receive family planning services. Pregnant women can get prenatal care and counseling. The goal is to ensure delivery of a healthy baby. Insurance plans including Medicare and Medicaid are accepted. If you do not have insurance, care is provided on a sliding fee scale. Please call your health center and speak with a Human Service Counselor.
