Transportation for the Elderly and Disadvantaged

This program provides transportation to the elderly and disabled. It allows clients access to health and medical care, shopping and social activities. It provides door to door service for those who do not have or cannot afford their own transportation. This includes wheelchair transport. Service is provided daily to various sites. In-county shopping trips are provided every Thursday. Private pay rates depend on the destination and trip miles.

There are no services on major holidays, except for dialysis patients.

Rates:
Medicaid Provider trips -$2.00 round trip
Transportation Disadvantage (TD) - $3.00 round trip
Private Pay – for those who do not qualify for Medicaid or TD; rates vary

Alachua, Baker, Columbia and Duval

Tri-Counties Care Center - Haven Hospice - Chiefland

This Care Center provides end-of-life care. It address the physical, social, spiritual and emotional care of each patient. There are services for family members as well. The Care Center is there for those who can no longer remain in their home. The 16-bed center provides all the comfort of home along with full hospice services. Victim Funds - MADD
