Haven Hospice - Transitions Program

Haven Hospice provides Transitions Program. This program offers emotionally supportive calls, volunteer companions, and referrals to community resources for additional support. These services are offered at no charge.

This program offers:

  • Supportive phone calls to individuals and caregivers.
  • Volunteer assistance for those who can benefit from companion visits.
  • Information about and referrals to community resources.
  • Education to help with understanding care options and choices.
  • Advance care planning inclusive of advance directives.

Healthy Start of Central & North Florida Coalitions - The MIECHV/Parents as Teachers Program

MIECHV / Parents as Teachers is free for parents with infants and young kids in stressful life situations. They teach parents and caregivers skills to help their kids be healthy and safe. As well successful in school.

Alachua and Marion counties: for kids up to age 3.
Bradford, Columbia, and Putnam counties: for kids up to age 5.

Service Animals - Canine Companions

People who are disabled due to injury or disease may qualify for an assistance dog. These highly trained dogs are able to assist with many tasks. There are 4 types of assistance dogs:

  • Facility - assists in an education or health care setting
  • Hearing – assists the hearing impaired
  • Service – assists with daily tasks
  • Skilled Companion – assists physically, mentally or developmentally disabled
