Or other community-based settings. The group does not help people directly but has several programs to help connect them with services that do. These programs help older adults and people with disabilities. Helps them find supports and services that are needed to successfully live in their communities. Haven Hospice provides a large range of health care services. Care is provided wherever the patient calls home. They serve the North Central Florida region.

Haven Hospice - Tri-Counties Care Center- Chiefland

This Care Center provides end-of-life care. It address the physical, social, spiritual and emotional care of each patient. There are services for family members as well. The Care Center is there for those who can no longer remain in their home. The 16-bed center provides all the comfort of home along with full hospice services. Jewish Family & Community Services - Emergency Financial Assistance

Jewish Family & Community Service helps with social and community services. They may be able to help with Emergency Financial Assistance. JFCS does not accept walk-ins for financial assistance.
