Advance Psychiatric Directives - Mental Health Association of Okaloosa-Walton Counties, Inc.

This program is for persons who might not be able to make their own treatment decisions some time in the future. Advance Directive is a document that lets you name a friend or family member (Health Care Surrogate) to carry out your wishes. It can be prepared without a lawyer. It must be witnessed by two adults. This is a free program.
