This Care Center provides end-of-life care. It focuses on the physical and social care of each patient, as well as the spiritual and emotional care. There are services for family members as well. This group offers Veterans Direct Care. It is called VDC for short. This program is for veterans who need help with daily living activities. It is also for clients who are isolated or who have a caregiver that is experiencing burden. There are eligibility requirements to use the program.
