Christian Care Center - The Community Medical Care Center

The Community Medical Care Center provides medical and dental services to low-income patients. The patient must earn less than 150 percent of federal poverty guidelines. They must be able to show documents for proof. Once the patient qualifies, their care is free. There are doctors from many different specialty fields volunteer at the clinic. The clinic operates with a substantial wait time, given the demand for services versus the supply available. If you have a medical emergency, contact 911 or see a primary care doctor.

In addition to primary care, the clinic also provides:

  • Chronic disease education and care
  • Referrals for free diagnostic tests
  • Referrals for surgical procedures in some cases

Medicare is a Health Insurance Program for people age 65 or older. Some disabled people under age 65 and people of all ages with End-Stage Renal Disease may also apply.

National Kidney Foundation of Florida, Inc. - Direct Aid Program

This agency helps dialysis patients with transportation costs not covered by other resources. Patients may use grants to travel to and from dialysis treatment, or for transplant work-ups. This is a funding of "last resort." Social workers must complete the NKF Application for Financial Assistance for the patient. Please see the website for more details.

National Kidney Foundation of Florida, Inc. - Hawthorne Patient Assistance Fund

The NFK offers the Hawthorne Patient Assistance Fund. It offers short-term financial help. Help is for qualified kidney transplant recipients. And is to help meet the cost of kidney transplant related medications. While the patient looks for other long-term solutions. The money is paid directly to the pharmacy of the patient’s choice. WellCare Florida Council - HIV Cares Program - AIDS Drug Assistance

The HIV Cares Programs, run by WellCare Florida Council, offers the AIDS Drug Assistance Program. It is also known as ADAP. This program helps HIV-positive people stay healthy. Helps them by assisting with the purchase of HIV prescription medication. They also provide drug treatments for people who do not qualify or are awaiting eligibility for Medicaid.
