Health Services - Medicaid Waiver/Home and Community Based Services

This program is for aged and disabled people who, without services, would be at risk of being in a nursing home. Eligibility for this program includes asset and income limits. Applicants must be eligible for institutional Medicaid. They must be 60 years old or disabled.

Baker County Council on Aging - Community Care for the Elderly

CCE provides services to elders 60 years or older. It is for those who need help with activities of daily living. Case managers are responsible for helping elders and family members find, plan and coordinate needed services and resources.
