ACL - Connecting People to Services

The Administration for Community Living or ACL for short. The group believes that services should be strong enough so individuals can be supported in their own home. Or other community-based settings. The group does not help people directly but has several programs to help connect them with services that do. These programs help older adults and people with disabilities. Helps them find supports and services that are needed to successfully live in their communities. Haven Hospice - Tri-Counties Care Center- Chiefland

This Care Center provides end-of-life care. It address the physical, social, spiritual and emotional care of each patient. There are services for family members as well. The Care Center is there for those who can no longer remain in their home. The 16-bed center provides all the comfort of home along with full hospice services. Medicare

Medicare is a Health Insurance Program for people age 65 or older. Some disabled people under age 65 and people of all ages with End-Stage Renal Disease may also apply.
