CARD helps by sharing lists of resources. This list is personalized to the client’s area and insurance. Consultations can be done over the phone, over email, or video chat. They can also meet in the community or in your home. This group works with people of all ages. All services provided are short term. This program offers supportive services. The help is for homeless individuals and families. Those being helped have been diagnosed with a severe and persistent mental illness. The program will help stabilize and prepare them for a life of independence. Program fees are based on income. There are requirements that must be met to participate in the program.
